This form requests certain basic information from schools (primarily contact information) and a list of items that the sanitarian will need to perform the inspection.  The inspecting sanitarian shall contact school officials in advance if they wish to be accompanied by a school staff member during the inspection.

School Inspection Form 1 and 2 – Final.pdf

This form will be returned to the school and will indicate all areas cited for remedial care.  The school’s response to these citations is a written plan submitted within sixty days of the receipt of the inspection report, although a local board of health may grant an extension for an additional thirty days.

 

Roof Inspection Form – Final.pdf

This form is to be completed by a properly trained individual who inspects the school’s roof either on an annual basis or after severe weather (as determined by the school district).  It is to be made available to the inspecting sanitarian at the time of the school inspection.
